Caloocan City’s Brgy. 118-120 Health Center has been transformed, evolving into a vibrant healthcare hub that is SM Foundation’s 204th Wellness Center.
The newly rehabilitated center continues the SM group’s commitment of creating spaces that not only address the healthcare needs of the community but also foster comfort and well-being.
A healthcare hub for 25,516 individuals
The 204th Wellness Center serves a thriving population of 25,516 residents.
It offers various services–from routine check-ups to specialized programs on family planning, immunization, nutrition, dental health, TB-DOTs, and cervical cancer screening.
A transformation beyond the structure
Through the initiative, SM Foundation has placed LED appliances, lighting fixtures, and new medical tools and equipment. The facility’s layout has been maximized, adhering to the Department of Health’s guidelines for patient safety.
The interior design has been updated to create an atmosphere distinct from traditional clinical settings, alleviating worries typically associated with healthcare facilities.
SM Foundation installs a Mobile Play Cabinet in the center to alleviate the apprehension of young patients during check-ups.
As SM Foundation strives for inclusivity, it continues to recognize the importance of a holistic approach to healthcare. To alleviate the apprehension of young patients during check-ups, a mobile play cabinet stocked with gender-neutral toys has been installed.
With the renovation, the center carries out its program and services, such, routine check-ups to specialized programs on family planning, immunization, nutrition, dental health, TB-DOTs, and cervical cancer screening, with ease.
A testament to the strength of social good collaboration
Built through the social good collaboration of the SM group, the Rotary Club of Caloocan-Monumento, Caloocan City Health Office, and the Caloocan Local Government Unit, the facility highlights the potential impact when different entities join forces for a crucial cause–the health and well-being of the community.

According to Dr. Arlyn Gonzales, Medical Officer IV and doctor in charge of the health center, there has been a steady uptick in patient visits following the completion of the renovation.
“Before the renovation, there were challenges. Our ceiling was damaged, our lights were dim, and our furnitures were old. Marami ring mga nakatambak. We also had no examination room. The renovation was truly very timely,” Dr. Gonzales said.
“When our patients visit now, they often say, ‘Wow! Ang ganda at liwanag na ng center.’ We also noticed that they maintain good hygiene before coming to the center. Some even visit regularly or come ahead of time even if it’s not yet their schedule for the follow-up checkup,” she added.
She also shared that the same enthusiasm is observed among health workers: “Now that the facility provides a conducive environment, we can enhance the quality and delivery of services to the community. We are enthusiastic about our duty and are genuinely happy to serve.”
The new Brgy. 118-120 Health Center is a symbol of hope for healthcare and collaboration. Alongside the physical transformation, it embodies the collective spirit of a community dedicated to ensuring health workers’ satisfaction and safety at work while ensuring the health and wellness of its residents.
